How to apply

Applications will only be considered from NHS staff. Direct applications are no longer accepted.
Applicants must be receiving treatment for Diabetes from a hospital in the UK
Age limit normally 19 years old
Applicants must be resident in the UK
Applicant’s family must be in receipt of means tested benefits (e.g. Universal Credit, JSA, ESA, or Income Support. Disability Living Allowance and Carers Allowance are not relevant).
Grants will not be available as a regular supplement to income and should only be applied for in times of crisis.

Application Form

Applications for patients must be submitted through a nurse or Dr. at the NHS, either through an NHS email address or written on NHS headed notepaper.

All patients should be on means tested benefits, to include Universal Credit (UC), Income Support (IS) and Employment and Support Allowance (ESA). Please note Disability Living Allowance (DLA) and Carers Allowance (CA) are irrelevant to the application. Please can the applicants check that their patients are on Means Tested benefits before applying.
